How to Correct Passport or Address Details on a Foreigner’s Policy

A misspelled name, an old passport number or an incomplete address can make an insurance document difficult to use. Report the discrepancy before relying on the policy for a visa, residence-card or other administrative matter. Compare the document with the purchase data and your current identity document, then send a clear request through the service route shown in the policy documents or provided by the intermediary. Ask what evidence is needed, what corrected document will be issued and how completion will be confirmed. Keep the original file unchanged, save the correspondence and retain any updated certificate or endorsement.

Check the policy before reporting an error

Begin with the document you received, not a name copied from an email subject or payment confirmation. Review the policy or certificate and identify the exact field that is wrong. Common discrepancies include:

  • a missing or misplaced part of a first name or surname;
  • spelling that does not match the passport;
  • an incorrect passport number;
  • a wrong date of birth;
  • an old, incomplete or mistyped address.

Compare it with the source document containing the correct data. For a name, passport number or date of birth, this will normally be the relevant identity document. For an address, use details you can support if asked. A transliteration, abbreviated address or different name order may not be an error, so ask how the data should appear.

Check whether the information appears more than once. Review the insured person’s details, policyholder details and any certificate intended for an office. If several fields are inconsistent, put them in one complete request rather than separate messages.

The 2020 Uniqa terms for this type of insurance state that the policyholder and insured person must disclose all circumstances known to them that the insurer asked about before the contract was concluded. The provision does not set a correction procedure, deadline or document list, but it supports the need to report a discovered discrepancy clearly.

Report the error step by step

1. Identify the contract

Give the policy or certificate number and the insured person’s name as it currently appears. This helps distinguish the relevant contract from another purchase or an earlier document.

2. Describe the discrepancy precisely

Name the field and state both versions: what the document shows and what it should show. For example, write that the passport number displayed on the certificate is incorrect, then provide the correct number. Avoid a vague message such as “my data is wrong”.

3. Use the stated service route

Follow the contact or servicing route shown in the policy documents, purchase confirmation or instructions supplied by the intermediary. If you purchased through an intermediary, ask whether the request will be handled there or passed to the insurer.

4. Ask what evidence is required

Say that you can provide supporting material and ask for the specific documents needed for this correction. Do not send unrelated medical or claim documents. A request to correct personal data is different from reporting an insured event.

5. Ask what corrected record will be issued

Request a clear answer on whether you will receive a revised policy, a new certificate, an endorsement or another form of confirmation. A new certificate is not guaranteed in every case, so ask what document will show the accepted data.

6. Save proof and follow up

Keep a copy of the request and attachment list. Ask for written confirmation of receipt and retain the reply. Ask when you can expect a response, and do not plan around an unconfirmed completion date. If the matter is time-sensitive, state when you expect to use the document without treating that date as a guaranteed deadline.

Important: Do not alter the insurance document yourself.

Editing a PDF or handwriting a correction does not create an insurer-issued record. Wait for the document or confirmation supplied through the servicing process.

Prepare the information that may be requested

The insurer or intermediary may need information to identify the contract, understand the discrepancy and verify the replacement data. Prepare:

  • the policy or certificate number;
  • the policyholder’s and insured person’s names as shown on the document;
  • the incorrect value and the requested correct value;
  • a short explanation of whether this is a typographical error, a renewed passport or a later address change;
  • a reliable email address or other return contact used for the policy.

The servicing party may ask for identity evidence, such as the relevant part of a passport, or evidence supporting the new data. A person submitting the request for someone else may need proof of authority to act. These examples are not a fixed list.

Ask which pages or fields are required and how to send them. Share only what is necessary for the stated purpose through the specified route. Documents used for a medical claim are not automatically required for a data correction.

Ask which supporting documents are needed for your particular correction. Before accepting any charge, ask whether a fee applies to your request.

If you need the insurance record with other formal documents, review documents used when extending a visa in Poland. Foreign employees can also check medical insurance documents for foreign workers. Follow the servicing instructions for your own contract.

Check and store the corrected documents

When a revised document or endorsement arrives, compare it with the source information. Inspect every repeated field, not only the detail reported. Confirm that the name, passport number, date of birth and address are consistent and that the policy number and insured person still match the contract.

If something remains wrong, reply in the existing correspondence thread. Identify the unresolved field and attach the earlier confirmation. This preserves a complete correspondence trail.

Save the insurer-issued corrected certificate or endorsement in its original format with the request, receipt confirmation and replies. Label any previous version clearly so that you do not submit it by mistake.

For administrative use, keep both the correspondence and updated insurance document. Neither guarantees acceptance by a visa office, residence authority, employer, university or another recipient. Check the receiving authority’s current requirements before submission.

Frequently asked questions

Can I correct the PDF myself before submitting it?

No. Use an insurer-issued document or confirmation rather than editing the policy file yourself. Send the discrepancy through the stated service route and ask what corrected record will be provided.

Is a copy of my passport always required?

Not necessarily. The servicing party may request identity evidence for a name, passport number or date-of-birth correction, so ask which document and which part of it are needed for your request.

Will the correction be completed immediately?

Ask the insurer or intermediary when you can expect the correction; do not assume it will be immediate. Confirm that the request was received, provide requested evidence promptly and follow up through the same route if necessary.

Will I always receive a new certificate?

Not necessarily. Ask whether the accepted change will be recorded in a revised policy, certificate, endorsement or another confirmation. Retain the version actually issued.

Can I use the correction email at an office while I wait?

Do not assume that correspondence will be accepted in place of an updated insurance document. Keep the email as part of your records, check the receiving authority’s current document requirements and read when a health insurance policy may need translation. Avoid relying on an unconfirmed substitute.

Is a policy-data correction the same as an insurance claim?

No. A correction request is not a claim for medical expenses or another insured event. Do not copy claim-document requirements into the correction process. Use the correction instructions supplied for your contract.

Correct the details before relying on the policy

A useful correction request identifies the contract, shows the incorrect and correct values, asks what evidence is needed and requests confirmation of the resulting document. Check the returned record and store it with the correspondence. This does not promise instant processing or acceptance by an authority, but it records the steps taken.
